Chronic AC - strain

Clinical Picture:

Pain is felt at the tip of the shoulder or in the C4 dermatome

Posttraumatic or as an overuse injury.

A special form of AC- strain is the " distal clavicular osteolysis "

The pain is felt during or after activity.

Pattern:

Full range of movement with pain at the end of ranges.

Sometimes in combination with a painful arc which locates the lesion in the inferior acromio-clavicular ligament.

Local tenderness at the AC-joint if the superior acromioclavicular ligaments are affected.

An additional test (passive horizontal adduction ) is usually the most painful movement.
