As an alternative for procaine

Whenever the injection is too painful ( acute bursitis).

In very sensitive patients.

To add with triamcinolone, so making the infiltrations less painful.

Lidocaine is also an alternative when an allergy for procaine is present or suspected.
The local anaesthetic effect is stronger so its concentration should be diluted to 0,5%. This is certainly so in diagnostic infiltrations.
Sometimes it is recommended to add a strong and quick acting local anaesthetic to the triamcinolone.

Dilute the triamcinolone when too large lesions must be infiltrated.
