The Shoulder

Crystal synovitis is due to urate crystals (gout) or pyrophosphate crystals (chondrocalcinosis) and is extremely rare at the shoulder

Consider this diagnosis when a capsular pattern comes on spontaneously and in a few hours to a few days.

There is a pronounced capsular pattern with a spastic end feel together with severe tenderness over the joint.
