The tendon of the subscapularis has a broad insertion on the lesser tuberosity and on the proximal extend of the minor tubercular crest, medial to the biceps groove.

The fibres run in a horizontal direction.

The cranial part of the tendon passes underneath the acromial roof to insert high up on the lesser tuberosity. On elevation, a momentary impingement of an upper lesion produces a painful arc .

The lower half of the subscapularis tendon inserts low on the tubercular crest and is brought in contact with the coracoid process during a passive horizontal adduction.
